We offer senior focused primary care designed to meet patients where they are. Our services include senior residential community visits, telehealth visits, and clinic visits. In addition, we provide advanced primary care management for eligible patients who benefit from closer coordination, proactive outreach, and ongoing support between visits.

We offer same-day and next-day telehealth and clinic visits for new and established advanced primary care management patients to handle acute needs. For facility visits, please call our office and our staff will coordinate scheduling based on when a provider is nearby, often within the same week.

Not at all! Our providers are happy to travel to your senior residential facility or see you virtually.

Advanced primary care offers more access and support than traditional primary care, including senior living facility visits and reserved slots for same-day and next-day telehealth and clinic appointments. It also includes ongoing care coordination between visits and 24/7 access through an after-hours support line.

No. While our practice is designed specifically for seniors, we are happy to see patients of other ages whose needs and insurance are a good fit for our care model.
